All surveys, no matter how small or specific, are subject to [http://www.winona.edu/faculty/3-7.pdf WSU Regulation 3.7 Policies and Procedures for the Use of Human Subjects in Research]. The regulations define research as, “a systematic investigation, including research development, testing, and evaluation, designed to develop or contribute to generalizable knowledge” and a human subject as, “a living individual about whom an investigator (whether professional or student) conducting research obtains (1) data through intervention or interaction with the individual, or (2) identifiable private information.” Questions as to whether or not your survey constitutes human subjects research may be directed to the Human Protections Administrator, Brett Ayers (Maxwell Hall 161, 507-457-5519, [mailto:bayers@winona.edu bayers@winona.edu]), or any faculty member of the [http://www.winona.edu/faculty/1172.asp Institutional Review Board (IRB)]. All faculty, students and staff conducting research involving human participants are required to submit a Protocol Request for WSU IRB Approval for review by the IRB prior to initiation of any such research. Overview

Qualtrics is a full-featured, web-based tool for creating and conducting online surveys. This tool is both easy to use and extremely powerful. Qualtrics enables users to do many kinds of online data collection and analysis including market research, customer satisfaction, and course evaluations. All WSU employees and students can create an Qualtrics account. Winona State's Qualtrics instance is located at: https://winona.edu/qualtrics.com. Do not use the free trial version at www.qualtrics.com.

Qualtrics Campus Use Policy

Winona State University has a Qualtrics licensing agreement for faculty, staff and students to create accounts. By default, all accounts are created with student-level permissions. These accounts will not be able to distribute surveys. Using Qualtrics for personal, commercial or other non-academic use is prohibited. Using Qualtrics for third-party, for-profit and not-for-profit organizations is not allowed.

It is important that you create your Qualtrics account on http://WINONA.qualtrics.com site (WSU Licensed) and NOT on the Qualtrics main page. By default, all accounts are created with student level permissions which do not have permission to distribute surveys.
